Cencora forecasts double-digit profit growth for specialty services in fiscal 2027. Its outlook for the whole company is much slower. The new reporting structure shows why the results can move at different speeds.

Research by Ticker Byline shows that specialty services generated about 35% of total segment operating income in the recast fiscal 2026 figures. That gives investors a clearer view of the business behind Cencora's next growth phase.

Specialty earns more per sales dollar

Specialty Pharmaceutical and Provider Services generated $1.69 billion of operating income on the company's segment measure. Its operating margin was 3.73%, compared with 0.96% at the much larger distribution division.

The specialty business combines services for complex treatments, physician practices and specialist logistics. Its results include OneOncology following the February 2 acquisition; fiscal 2027 brings a full year of that business.

Total segment operating income was $4.81 billion after transactions between segments were eliminated. That measure excludes items such as intangible amortization and acquisition-related costs. Consolidated GAAP operating profit was $3.93 billion.

Why the group forecast is slower

BusinessExpected growth
Specialty services18% to 21%
Distribution and services2% to 4%
Cencora overall4% to 6%

The distribution division's lower growth rate matters because of its size. Planned exits also reduce the contribution from Other, which is forecast to earn about $200 million against $393 million in fiscal 2026.

The outlook assumes Cencora sells its Profarma stake in the first fiscal quarter and completes the MWI Animal Health merger with Covetrus on March 31, 2027.

Funding the expansion

Cencora issued $3 billion of senior notes and took out $1.5 billion of term loans to help fund OneOncology. At September 30, cash was $4.88 billion, short-term debt $203.2 million and long-term debt $11.06 billion.

Operating cash flow less capital spending produced $3.88 billion. Cencora's adjusted free cash flow was $3.808 billion after removing $73 million of antitrust settlement receipts. Its fiscal 2027 forecast is about $3.5 billion on that adjusted basis.
